Gifts are due by December 1st to reach their destination in a timely manner. Once this form is complete, await an email from a HAPCAP staff member with information from your tags. A tag(s) will be selected for you and provided via email.
If you can no longer provide a gift, please contact info@hapcap.org ASAP so that we may reassign your tag.
