Hocking Athens Perry Community Action is offering an incentive program for Perry County residents who receive the Covid-19 vaccination(s). To be eligible for the $50 Kroger Gift Card, please complete the application below and present your vaccination card indicating you have received one dose of the Johnson & Johnson vaccine or 2 doses of the Moderna or Pfizer vaccine on or after January 10, 2022. There is a limited supply of gift cards and they are available on a first come first serve basis.The funding for this program was provided through a grant from the United Way of Muskingum, Perry, and Morgan Counties.
